Encourage patients to remain at home
The La Vernède day hospital is a medical structure for outpatient care that receives patients aged 60 and over following surgery, an acute medical event or in the context of a chronic pathology. “In partnership with the Toulouse University Hospital, the clinic integrates the ICOPE program (integrated care for the elderly) developed by the WHO to prevent frailty and dependence in these patients. As such, the day hospital offers a fragility course, an innovative project on the territory which allows screening, a medical evaluation consultation as well as on-site care at La Vernède”, explains the director. The patient journey is spread over two half-days per week, with three 45-minute workshops per morning. “We work in partnership with local authorities, professional territorial health communities (CPTS) and nursing homes”, insisted on specifying Aurélie Bellanti. In addition to medical and rehabilitation care for seniors, the day hospital also works with patients with moderate or severe obesity, disabilities or comorbidities. And this, since December 2022. This unit of Conques-sur-Orbiel is part of the course of medico-surgical management of obesity associated with the hospital center of Carcassonne. Here again, the course is spread over several weeks – eight – at the rate of three afternoons per week.
To make all this possible, a cell of 400 m2 was created. In this one, a therapeutic apartment for patients to practice coping as well as possible with their weaknesses at home: cooking, taking a shower, a bath… But also individual care areas and a technical platform for rehabilitation consisting of all the necessary equipment: parallel bars, electric stand-ups, rehabilitation bikes, etc.
The opening of the La Vernède day hospital will have required the creation of 6.5 paramedical posts: physiotherapist, re-educator, psychologist, nurses… A physiotherapist post is still to be filled.
